I think that you need to get the VIN number from the car, then you can go to the stealership and get the code. Either that, or take the car into a stealership and they can reset the code for you.

Trunk

Did you look in the trunk on the hinge. Drivers side hinge, it should be a sticker with 5 numbers.

Worst case, there should be a sticker on the unit itself with the 5 digit code.
It's by the fuel cut-off unit in the driver side trunk behind the carpet.
 
Another way to get the factory preset code is by opening up the module and noting how the 5 parallel resistors are wired.
